Alcami’s Sample Submission Form contains all of the information you’ll need to log in your sample, initiate a variety of testing options and create a tailored Test Report or Certificate of Analysis. This form is designed to help Alcami understand and collaborate with our customers more closely, identifying who should receive the results, what the sample material is, what tests are to be performed and when the results are due. It also allows you to add comments including any special instructions for your unique materials.

FAQ

Why do you require so much information for me to submit my samples?

Alcami has a large diversity of clients submitting a variety of different sample types. Our login and laboratory teams need sufficient information to be able to clearly understand what you would like to be tested. We want RIGHT FIRST TIME for our sample logins allowing testing to begin quickly upon sample receipt. Many of the items are drop-down options allowing quick entry of the required information.

The old pdf sample submission form frequently missed information requiring delays in the login process resulting in delays in initiating testing and providing you results. Often very old or outdated forms were used with hand written information making it difficult to read and difficult to have a harmonized sample login form.

This form is confusing and very different than what I was used to in the pdf form. Are there instructions for using it?

Alcami’s new sample submission form is very similar to online surveys with a Stepindicator, progress bar and Previous and Next buttons. It has allowed us to show you what information we REQUIRE (denoted by the red Asterisks) to expedite the sample login process and prevent submission errors.

Why can’t I save my contact and payment information making multiple sample submissions easier and quicker?

There is a “Save and Continue Later” button at the bottom right of every step of the form that will save your CURRENT submission information by clicking it, entering your email address, accessing the email from Alcami and clicking on the emailed link.  This saves all information up to the point of clicking the button.

Currently, once you submit your sample submission, the link will reset and take you back to the beginning of the form.  Due to this and client feedback for this save feature being a high priority,  Alcami is expediting work on a new version of the sample submission form that will allow client login to a “portal”.  With this login feature, the site will be able to recognize your login and store your “profile” with this type of information.  We expect the new version to be ready about 3Q2016.

My computer freezes when trying to attach files or print the Sample Submission Form and I have to repeat entry.

Alcami recommends using the “Save and Continue Later” button at the bottom right of every step of the form that will save your CURRENT submission information by clicking it, entering your email address, and clicking Send Email. This will help save your Current information during the sample submission process. Currently, once you submit your sample submission, the link will reset and take you back to the beginning of the form.

Please ensure you are using the most up to date web browsers available. Alcami’s sample submission form is not web browser specific and will work on many types of browsers out there. There are some advantages to using, for example, Google Chrome, and Enabling Auto-fill for form entry to help expedite entry of some of the standard and required pieces of information.

Please ensure your documents that you would like to attach are NOT MORE THAN 2 MB. The form allows up to 2MB items (each) attached, so if your items are larger than this, you may need to break them up into multiple attachments (for example, print to PDF in sections of a few pages each).

Where do I indicate how many tests to perform?

Steps 3 and 4 contain fields for entry of samples you are sending us and what tests, including how many times to perform each test, for each sample or set of samples. Those pages look like this:
